  • Your answers are private
Step 3: Receive your NAQ Foundations Symptom Burden Graph
  • Once your questionnaire is submitted, you’ll receive an email with a copy of your NAQ Foundations Symptom Burden Graph
  • This usually arrives within 1 hour, and no later than 24 hours
  • Bring that to the webinar to understand your primary readiness signal when we talk about it live
